MBBS in Germany — officially called Medizin or Humanmedizin (Human Medicine) — is tuition-free at all public universities. Indian students pay only a semester contribution of EUR 150 to EUR 350 (INR 16,500 to INR 38,400) per semester. Total yearly cost, including living expenses: INR 11 lakh to INR 13.8 lakh at a public university. That is less than most private medical colleges in India, with a globally ranked degree at the end of it.
Here is what most guides do not tell you upfront: the entire program is taught in German. You need B2 to C1 German proficiency before admission. For most Indian students, that means 12 to 18 months of dedicated language preparation before you can even apply. Factor that in before you decide.
Germany has 35 medical universities ranked in the QS World Rankings for Medicine 2026. Charite Universitatsmedizin Berlin is ranked 18th globally for Medicine — the highest-ranked German medical university as per QS Medicine 2026. After graduating, doctors in Germany earn an average of EUR 68,300 per year (INR 75 lakh). The break-even on your total 6-year investment is under 2 years of working as a doctor in Germany.
NEET is mandatory for Indian students — not because German universities require it, but because the NMC (National Medical Commission) mandates it for any Indian student studying medicine abroad. Minimum 60% in NEET is required. Only about 5% of medical seats are reserved for non-EU students — making this one of the most competitive medical admissions in the world.

MBBS Course Duration and Structure in Germany
The Medizin (Humanmedizin) program in Germany takes 6 years and 3 months to complete.
- Students sit the Staatsexamen (State Examination in Medicine) at the end of the program.
- After passing, they apply for the Approbation — the permanent medical license issued by the state health authority (Landesbehorde).
- The title earned is Arzt / Arztin (Doctor of Medicine), which is the German equivalent of MBBS for international purposes.
If your German is below B2 when you apply, add 1 year for the M-Kurs at Studienkolleg — bringing the total to 7 years and 3 months. The program has four distinct phases each ending with a formal examination.
| Phase | Duration | What Happens | Exam at End |
|---|---|---|---|
| Pre-Clinical Study (Vorklinik) | 2 years (4 semesters) | Anatomy, physiology, biochemistry, medical mathematics — entirely academic, no patient contact | Physikum (Ärztliche Prüfung — First Medical Examination) |
| Clinical Study (Klinik) | 3 years (6 semesters) | Clinical subjects, hospital rotations, and direct patient care under supervision | Second Medical Examination (Zweiter Abschnitt der ärztlichen Prüfung) |
| Practical Year (Praktisches Jahr) | 1 year | Full-time supervised hospital training — surgery, internal medicine and one elective department | Final Staatsexamen (Third Section of the State Examination) |
| Approbation (Medical License) | 3 to 6 months after Staatsexamen | Application to state health authority (Landesbehorde) — submit degree certificate, language certificates and identity documents. Approbation is permanent, valid for life and recognised across all EU countries | No exam — document-based assessment |
After receiving the Approbation, graduates can specialise through Facharztausbildung (specialist training) — 5 to 6 additional years in a chosen field. Popular specialisations include surgery, cardiology, neurology, paediatrics, psychiatry and orthopaedics. Completing a specialisation significantly increases earning potential — specialist doctors (Facharzt) earn EUR 80,000 to EUR 95,000 per year (INR 87.9 lakh to INR 1.04 crore).
Studienkolleg and M-Kurs: What Indian Students Must Know
Germany follows a 13-year school system (10+3). India follows 10+2. This means most Indian students' Class 12 qualification is not directly equivalent to the German Abitur and cannot be used to apply directly to a German medical university. The solution is the M-Kurs at Studienkolleg — a 1-year preparatory course specifically for students targeting medicine, biology or pharmacy programs.
How the M-Kurs Works
- The M-Kurs covers German language, biology, chemistry, physics, mathematics and medical terminology
- Admission requires passing an entrance test (Aufnahmeprufung) in German and mathematics — a German B1 to B2 level is needed to pass
- After completing the M-Kurs, students take the Feststellungsprüfung (FSP) — a comprehensive exit exam
- Your FSP score, combined with your Class 12 marks, determines your eligibility for medical school admission through the Numerus Clausus (NC) system
How Competitive Is Studienkolleg Admission?
M-Kurs seats are extremely limited. Most Studienkollegs have 20 to 40 seats per intake for the medicine track. Competition from international students is intense. A score of 70% and above in the Aufnahmeprufung is generally required to secure a seat.
What FSP Score Do You Need for Medical School?
The FSP is graded on a scale of 1.0 (best) to 4.0 (minimum pass). For medicine, most universities require an FSP score of 1.0 to 1.5. Combined with a Class 12 aggregate of 85% and above across all six subjects, this FSP score determines your NC position. German universities consider marks from all six Class 12 subjects — not just the best four as in India. A weak score in any subject including languages reduces your aggregate and your NC position.
Eligibility and Admission Requirements for MBBS in Germany
Most German medical universities open applications for the winter semester between April and August. Here are the full eligibility criteria for Indian students applying for the Medizin program in Germany in 2026.
Academic Requirements
- Completed Class 12 from a recognised board with Physics, Chemistry and Biology
- Minimum 85% aggregate across all six Class 12 subjects — not just PCB
- NEET qualification with a minimum 60% — mandatory as per NMC regulations for all Indian students studying medicine abroad
- Eligibility Certificate from the Medical Council of India
- Studienkolleg M-Kurs completion with an FSP score of 1.0 to 1.5 for competitive universities
- 1 to 2 years of voluntary or community medical service work — required by some universities, including Heidelberg
German Language Requirements
| Language Test | Required Level | Notes |
|---|---|---|
| TestDaF | Level 4 (TDN 4) in all four skills | Most widely accepted across all German universities |
| DSH (Deutsche Sprachprüfung für den Hochschulzugang) | DSH-2 or DSH-3 | Taken at German universities — requires prior B2 level to attempt |
| Goethe-Zertifikat | C2 level | Accepted by most universities |
| KDS or GDS | C2 level | Accepted by select universities |
B2 is the minimum for admission. C1 is strongly recommended for clinical rotations where you interact directly with German-speaking patients. C2 is required for the Approbation in most German states — plan for this level from the start of your language preparation.
Documents Required
- Online application form via Uni-Assist or university portal

Important note on TUM: The Technical University of Munich introduced non-EU tuition fees of EUR 2,000 to EUR 6,000 per semester from winter semester 2024/25. Bottom line: Total yearly cost at a public university outside Baden-Wurttemberg = INR 11 lakh to INR 13.8 lakh. Over the full 6-year program, total investment is approximately INR 66 lakh to INR 82.8 lakh — compared to INR 50 lakh to INR 1.2 crore at private medical colleges in India.

Jobs and Salary after MBBS in Germany
Germany has a documented shortage of doctors, particularly in rural areas and among ageing populations. According to the German Federal Statistical Office (Destatis), Germany needs approximately 50,000 additional doctors by 2035. This makes it one of the most favourable countries in the world for medical graduates seeking employment immediately after graduation.
After completing the Medizin program and passing the Staatsexamen, graduates apply for the Approbation from the relevant state health authority (Landesbehorde). Submit your degree certificate, language certificates and identity documents. Processing takes 3 to 6 months. The Approbation is permanent, valid for life and recognised across all EU member states — no additional exam required to practice in any EU country.

FAQs on MBBS in Germany
Ques. Is MBBS free in Germany?
Ans. Yes. The Medizin (Humanmedizin) program — the German equivalent of MBBS — is tuition-free at all public universities in Germany. Indian students pay only a semester contribution of EUR 150 to EUR 350 (INR 16,500 to INR 38,400) per semester covering administrative costs and student services. The exception is universities in Baden-Wurttemberg state such as Freiburg which charge non-EU students EUR 1,500 per semester as tuition. Private universities charge EUR 5,000 to EUR 20,000 per year. Conversion: 1 EUR = INR 109.83 as of April 14, 2026.
Ques. What is MBBS called in Germany?
Ans. MBBS is not a degree that exists in Germany. The equivalent program is called Medizin or Humanmedizin (Human Medicine). Students sit the Staatsexamen (State Examination in Medicine) at the end of the program. After passing, they receive the Approbation — the permanent medical license — and earn the title of Arzt / Arztin (Doctor of Medicine). The Approbation is the German equivalent of MBBS for international purposes and is recognised across all EU member states.
Ques. How many years is MBBS in Germany?
Ans. The Medizin (Humanmedizin) program in Germany takes 6 years and 3 months to complete — 2 years of pre-clinical study (Vorklinik), 3 years of clinical study (Klinik) and 1 year of practical training (Praktisches Jahr). If your German proficiency is below B2 when you apply, add 1 year for the M-Kurs at Studienkolleg, bringing the total to 7 years and 3 months. Most Indian students take the longer route since German language preparation from scratch requires 12 to 18 months.
Ques. Is MBBS from Germany valid in India?
Ans. The Medizin degree from Germany is not directly recognised by the National Medical Commission (NMC) in India. To practice medicine in India, you must pass the Foreign Medical Graduate Examination (FMGE) or the NExT exam. The FMGE pass rate for Germany graduates is approximately 40% to 50% — significantly higher than the overall average of 20% to 25% for all foreign medical graduates. There is no limit on the number of FMGE attempts.
Ques. Does Germany require NEET for MBBS?
Ans. German universities themselves do not require NEET. However, NEET is mandatory for Indian students as per NMC (National Medical Commission) regulations. Any Indian student who studies medicine abroad without qualifying NEET cannot register as a doctor in India. You need a minimum of 60% in NEET to be eligible for medical studies abroad. This rule applies regardless of which country you study in.
Ques. Can I study MBBS in Germany in English?
Ans. No. The Medizin (Humanmedizin) program at German public universities is taught entirely in German. There are no English-medium MBBS equivalent programs at public universities. A small number of private universities offer medicine in English but charge EUR 5,000 to EUR 20,000 per year in tuition. For Indian students who want an English-medium medical degree in Europe, Hungary, Poland and the Czech Republic are the main alternatives worth considering.
Ques. How many seats are available for Indian students in MBBS in Germany?
Ans. German medical universities reserve approximately 5% of total seats for non-EU international students. With roughly 10,000 total medicine seats across all German universities per year, this means around 500 seats are available for all non-EU students combined — not just Indians. Competition is extremely intense. Most successful Indian applicants have FSP scores of 1.0 to 1.5 and Class 12 aggregates of 90% and above across all six subjects.
Ques. What is the total cost of MBBS in Germany for Indian students?
Ans. The total yearly cost of the Medizin program in Germany for Indian students at a public university outside Baden-Wurttemberg is approximately INR 11 lakh to INR 13.8 lakh. This includes the semester contribution (INR 16,500 to INR 38,400 per semester) and living expenses (INR 92,260 to INR 1,38,390 per month). There is no tuition fee at public universities outside Baden-Wurttemberg. Over the full 6-year program, the total cost is approximately INR 66 lakh to INR 82.8 lakh. Conversion: 1 EUR = INR 109.83 as of April 14, 2026.
Ques. What is the salary after MBBS in Germany?
Ans. The average starting salary for a doctor in Germany (Assistenzarzt) is EUR 55,000 to EUR 65,000 per year (INR 60.4 lakh to INR 71.4 lakh). Specialist doctors (Facharzt) earn EUR 80,000 to EUR 95,000 per year (INR 87.9 lakh to INR 1.04 crore). Senior doctors (Oberarzt) earn EUR 100,000 to EUR 120,000 per year (INR 1.10 crore to INR 1.32 crore). Conversion: 1 EUR = INR 109.83
Ques. What is the minimum Class 12 percentage required for MBBS in Germany?
Ans. Most German medical universities require a minimum aggregate of 85% across all six Class 12 subjects. Unlike India where only the best four subjects are counted, German universities consider all six subjects in your Class 12 result. For the most competitive universities like Charite Berlin and Heidelberg, a 90% and above aggregate is strongly recommended. A low score in any subject including languages reduces your eligibility score in the Numerus Clausus (NC) system.
Ques. What happens if I fail FMGE after completing the Medizin program in Germany?
Ans. If you fail the FMGE after completing the Medizin program in Germany, you cannot practice medicine in India until you pass it. There is no limit on the number of FMGE attempts. The FMGE pass rate for Germany graduates is approximately 40% to 50%. Alternatively, you can continue practicing medicine in Germany where your Approbation is fully valid, or in any other EU country without any additional exam. You can also clear USMLE for the USA or PLAB for the UK.
Ques. Is Germany good for MBBS?
Ans. Yes — for students who are willing to learn German. Germany offers tuition-free Medizin programs at globally ranked universities (Charite Berlin is ranked 18th in the world for Medicine by QS 2026), a clear path to EU permanent residency through the EU Blue Card and average starting doctor salaries of EUR 55,000 to EUR 65,000 per year (INR 60.4 lakh to INR 71.4 lakh). The main challenges are the German language requirement (B2 to C1), the limited non-EU seats (approximately 5% of total) and the need to pass FMGE/NExT to practice in India.
Ques. Can I do MBBS in Germany after 12th?
Ans. Yes. You can apply for the Medizin (Humanmedizin) program in Germany after completing Class 12 with Physics, Chemistry and Biology. However, most Indian students must first complete a 1-year M-Kurs at Studienkolleg since the Indian 10+2 qualification is not directly equivalent to the German Abitur. You must also achieve B2 to C1 German proficiency and qualify NEET before applying. The realistic timeline from Class 12 to starting medical school in Germany is 2 to 3 years.
Ques. What are the disadvantages of studying MBBS in Germany?
Ans. The main disadvantages of studying the Medizin program in Germany for Indian students are: the German language barrier (B2 to C1 required — 12 to 18 months of preparation for most Indians), extremely limited non-EU seats (approximately 5% of total — around 500 seats for all non-EU students combined), the need to pass FMGE/NExT to practice in India, a complex multi-step application process involving Studienkolleg and Uni-Assist, and higher cost of living compared to Russia or the Philippines. The degree is not directly recognised by NMC India — FMGE/NExT is mandatory to practice in India.
