As per NTA released exam day guidelines, along with the NEET Admit Card, candidates have to carry a self-declaration form (Released along with the admit card) to the NEET Exam Center. The NEET 2024 admit card was released on May 02, 2024, and students can download it from the official website.

Candidates from UT of J&K and Ladakh must submit an online Self-Declaration for seats under 15% All India Quota. Furthermore, all students must keep the self-declaration for future reference, as the authorities will verify it during the counseling process. Also, any fabrication or manipulation of the self-declaration form can lead to disqualification.

Step 1: Log in to the official NEET website - exams.nta.ac.in/NEET

Step 2: Click on the link: NEET UG 2024 admit card

Step 3: Then, enter the application number and date of birth and download the NEET hall ticket 2024.

Step 4: Fill in the details in the NEET 2024 self-declaration form manually.

Step 5: Lastly, attach a photograph in the relevant space.

The self-declaration form contains health-related information, and students have to declare -

  • Absence of COVID-19 Symptoms: Students must confirm that they do not have any symptoms related to COVID-19. The symptoms may include fever, cough, or cold on exam day.
  • No Exposure to COVID-19: Also, students have to confirm that they have not come in contact with any individuals who have tested positive for COVID-19.
  • Adhering to Exam Rules: Lastly, the form will have a statement concerning that the student will follow all the rules in the examination hall and they will need to acknowledge the same.

What is the NEET 2024 Admit Card Undertaking Declaration?

The NEET 2024 admit card undertaking states, “I have read the instructions, guidelines, and information bulletin, instructions and notices related to this examination available on the website: neet.ntaonline.in and exams.nta.ac.in/NEET/. I have read the detailed 'important instructions for candidates' as given on page 3 and I undertake to abide by the same."

The students must sign this declaration on the NEET 2024 admit card undertaking. By signing this declaration, students are self-declaring that they will abide by the NEET UG guidelines and instructions.

Besides signing the undertaking, candidates should also affix their photo which should be identical to the NEET application form photo. Students should also note they need a left-hand thumb impression and a parent’s signature.

Reporting Time: Students must reach the exam center before the reporting time to avoid delay. Note that no students are allowed to enter the Exam Center 2024 after the gate closing time.

Items Allowed: Apart from mandatory items, candidates can carry A transparent water bottle, a Transparent hand sanitizer bottle additional photographs, PwD certificates, and scribe-related documents (if applicable).

Prohibited Items: Candidates must not carry any of the prohibited items. A few of the prohibited items include stationery, electronic devices, food items, accessories like jewelry, and bags, etc.

Hence apart from bringing the admit card, students must also carry the undertaking and other essential documents to the examination hall. It is important to remember that must sign the declaration and should prohibit themselves from bringing any banned items.
