NTA will release the NEET Self Declaration Form on May 1, 2025 on the official website along with the NEET Admit Card.

Candidates belonging to UT of J&k and Ladakh have to submit their self declaration form online for seats under the 15% All India Quota confirming that they will be participating in the All India Quota Counselling.

The candidates who belong to different UT or states have to provide a domicile certificate if they wish to avail the benefits of the 85% state quota seats.

The NEET Self Declaration Form consists of the candidate's personal details and exam day instructions. The candidate should not sign the form prior to the exam day, it is to be signed in the presence of an invigilator.

Is Self Declaration Form mandatory in NEET?

NTA has made it mandatory for the candidates to carry the NEET Self Declaration Form along with the NEET 2025 Admit Card on the exam day. This ensures that there is a smooth conduct of the NEET 2025 exam as

  • The NEET Self Declaration Form is a candidate’s confirmation that they have read the examination guidelines mentioned on the Admit Card and agree to follow them.
  • The Self Declaration Form is also used during the NEET Counselling process so the students must keep their forms safely even after the exam is concluded.

How to fill the NEET 2025 Self Declaration Form?

Candidates should first download the Self Declaration Form which is attached with the NEET 2025 Admit Card from the official website by logging in using their application number and password.

  • Read all the mentioned exam day guidelines and instructions thoroughly before filling out the form to avoid any mistakes or confusion.
  • Fill in the personal details, attach a photograph along with the candidate’s and guardian’s signature.
  • A print of the candidate’s left hand thumb is also required to complete the form filing.
  • Candidates must not sign the Self Declaration Form before the exam, the form has to be signed in front of an invigilator.

What are the NEET 2025 Exam Day Instructions?

The NEET 2025 exam day instructions are mentioned in the NEET 2025 Admit Card. Candidates must check and thoroughly go through them:

Reporting Time: The NEET 2025 exam starts at 2PM on May 4. Candidates must report to the exam centre before 1PM. Once the gates are closed the candidates strictly will not be allowed entry in the examination hall.

Dress Code: All candidates must wear light colour clothes and avoid any clothes with big buttons or buckles. Only slippers and sandals are allowed in the examination hall, shoes are not allowed.

Prohibited Items: Candidates are strictly not allowed to carry items such as electronic devices, stationary, food items, study material and jewellery to the exam hall.

Items Allowed: Candidates are only allowed to carry a transparent water bottle, hand sanitizer and mandatory documents along with them.
