MBBS abroad means pursuing a Bachelor of Medicine and Bachelor of Surgery degree at a foreign university outside India. The degree is awarded by a foreign university and is recognised for medical practice in India — but only after clearing a licensing exam called the FMGE (Foreign Medical Graduate Examination), completing two internships and meeting all NMC conditions.

Every year, approximately 25,000 Indian students leave for countries like Russia, Georgia, the Philippines, Kazakhstan and Bangladesh to study medicine. The reason is straightforward: India has roughly 1.28 lakh MBBS seats for over 22 lakh NEET applicants. Government seats are fiercely competitive. Private college fees in India range from INR 60 lakh to INR 1.5 crore for the full course. MBBS abroad fees is at INR 20 to 45 lakh total, which for most middle-class families is the only realistic path to a medical degree.

But here is what most guides do not tell you upfront: only 18.61% of foreign medical graduates passed the FMGE in June 2025. NMC Rules for MBBS Abroad in 2026

The NMC does not care where you studied. It cares whether you followed the rules. If you did not, your degree cannot be used to practice in India.

The National Medical Commission (NMC) governs MBBS abroad through the Foreign Medical Graduate Licentiate (FMGL) Regulations 2021. These are non-negotiable. Understanding them before you choose a country or university is the single most important step in this process.

The 54-Month Rule and the Two-Internship Requirement

Most students are told they need a "5-year MBBS." That framing causes problems. The NMC requires exactly 54 months of academic instruction — not 4 years, not approximately 5 years. Programs shorter than 54 months are not recognised for Indian licensing, regardless of what the university or consultancy tells you.

Two internships. Not one. The foreign internship happens before you return to India. The Indian internship — called CRMI (Compulsory Rotatory Medical Internship) — happens after you clear the FMGE. Budget for both in your timeline and finances. Most guides and consultancies do not mention the second internship upfront.

NEET Is Mandatory — No Exceptions

Some foreign universities will admit you without a NEET score. They are allowed to. But if you skip NEET, you cannot sit for the FMGE. Your degree will exist but cannot be used to practice medicine in India. If your goal is to practice in India, NEET is not optional.

Minimum NEET requirement: qualifying score with at least 50% aggregate in Physics, Chemistry and Biology (General category); 40% for SC/ST/OBC. NEET scores are valid for 3 years from the date of result.

The "NMC Approved University" Myth

The NMC does not approve or certify individual foreign universities.

When a consultancy tells you a university is "NMC approved," they are either confused or misleading you. The NMC sets compliance rules. Verifying whether a university meets those rules is your responsibility. Check the WHO AVICENNA Directory (avicenna.ku.dk) independently before paying any fee.

Russia

Russia is where the largest number of Indian MBBS students study, with an estimated 10,000 to 12,000 new enrollments per year.

  • The overall FMGE pass rate of 29.54% is above the June 2025 national average but masks wide variation between universities.
  • Top institutions like Kazan State Medical University and Sechenov University show pass rates of 35 to 40%. Bottom-tier universities fall below 15%.

Georgia

Georgia has the highest FMGE pass rate among all major MBBS abroad destinations at 35.65% (NBEMS 2024 data).

  • The entire program, including theory and clinical rotations, is taught in English.
  • Georgian universities have actively aligned their curricula with NMC requirements, which directly improves FMGE outcomes.

One important caveat: completing MBBS in Georgia does not automatically make you eligible for a Georgian medical licence. The Georgian licensing exam is a separate process that most Indian graduates do not pursue. Whether this satisfies the NMC's country-licensing requirement is a grey area. Verify this directly with the NMC before enrolling. Tbilisi State Medical University (TSMU) is the most established option — recognised by NMC India, PM&DC Pakistan and FAIMER. Philippines

The Philippines is the only Asian country where the medical curriculum follows the United States system. English is the national language — there is no language barrier in theory or clinical training. This directly contributes to a 24% FMGE pass rate, above the June 2025 national average.

Kazakhstan and Bangladesh

Both are rising middle-ground options. Kazakhstan's FMGE pass rate of 18.50% is improving year on year. Kazakh National Medical University in Almaty is the strongest performer. Kyrgyzstan
